ADDENDUM

The Quantity and Unit (“1 Lot”) of Item III. Venue Enhancements, Logistics and Incidental Requirements has been DELETED in the following sections:

1. Section VI. Schedule of Requirements, Quantity and Unit Column (page 61)

2. Section VIII. Bid Form, Table A, Quantity and Unit Column (page 72)

The “A. LOGISTICS AND SITE WORKS” of Item III. Venue Enhancements, Logistics and Incidental Requirements includes Hauling, Installation, Dismantling and Manpower in the following sections:

1. Section VI. Schedule of Requirements, Description Column (page 61)

 2. Section VIII. Bid Form, Table A, Requirement Column (page 72)

In Section VIII. Bid Form, Table A, Requirement Column, Item III. Venue Enhancements, Logistics and Incidental Requirements the phrase below was changed:

CLARIFICATORY

Section II. Instruction to Bidders, Clause 20. Sealing and Marking of Bids

Each Bidder shall submit three (3) sets of envelopes:

• (1) one ORIGINAL
• (2) two Photocopies stamped (Certified True Copy)

Each envelope must contain two (2) sets of the following bid documents both enclosed in another envelope:

• TECHNICAL COMPONENT
• FINANCIAL COMPONENT

Section II. Instruction to Bidders, Clause 12 and 13, Technical and Financial Component of the Bid

As stated in No. 12 & 13 of Section II. Instruction to Bidders of the Philippine Bidding Document the envelopes shall contain the following:

(1^st) First Envelope

TECHNICAL COMPONENT

(2^nd) Second Envelope

FINANCIAL COMPONENT

i.    PhilGEPS Certificate of Registration and membership – PLATINUM MEMBERSHIP

     (with the attached Annex A)

***In case any of the documents in the Annex A are already expired, an updated document must be submitted

i. Financial Bid Form on the Section VIII. Bidding Forms of the Philippine Bidding Document, which includes bid prices and the applicable Price Schedules, in accordance with the Section II. Instruction to Bidders Clauses 15.1 and 15.4

ii.      Statement of Ongoing Government and Private Contracts

iii.     Statement of Single Largest Completed Contract

iv.     NFCC Computation

v.      If applicable, the Joint Venture Agreement (JVA) in case the joint venture is already in existence, or duly notarized statements from all from all potential joint venture partners in accordance with Section 23.1 (b) of the IRR.

vi.     Bid Security in accordance with Section II. Instruction to Bidders, Clause 18

       Bid Securing Declaration using the form prescribed in Section VIII. Bidding Forms of Philippine Bidding Document.

vii.   Conformity with Technical Specifications, as enumerated and specified in Section VI. Schedule of Requirements and Section VII. Technical Specification of the Philippine Bidding Document.

viii.  Omnibus Sworn Statement in accordance with Section 25.3 of the IRR and using the form prescribed in in Section VIII. Bidding Forms of the Philippine Bidding Document.

ii.   Price list of Possible Incidental Items or Services as listed in Section III. Bid Data Sheet ITB Clause Column 15.4(a)(iv) and Section V. Special Conditions of Contract GCC Clause Column 6.2. (Bidders may add items on top of the listed items and/or services in the above sections)
