1. Bank’s Sworn Statement that the Bidder has an existing ATM based payroll remittance system as stated in Section 13.1 of the Bid Data Sheet 2. Detailed Breakdown of the Bid for the Janitorial Personnel and Summary of Supplies Requirement as attached in the Bidding Document Section VIII (Bidding Forms). NOTE: Bid Computation for Janitorial Personnel shall be based on DOLE Wage Order No. 21. The wage increase based on DOLE Wage Order No. 22 shall be adjusted during contract implementation. |
