Eligiblity Documents

Mayor’s Permit

In compliance with the GPPB Resolution No. 02-2015, the Procuring Entity allows the submission of the bidder’s recently expired mayor’s permit and the Official Receipt as proof that the bidder has applied for renewal of the permit within the prescribed period; Provided that, the current and valid mayor’s permit, as renewed, be submitted or presented by the bidder with the Lowest Calculated and Responsive Bid prior to award of contract.

Latest Income/Business Tax Return

Interested bidders shall submit 2017 Latest Income/Business Tax Return, stamped and received by BIR.

Qualitative Evaluation Documents

List of Awards and Recognition

List of Awards and Recognition for design and /or creative services undertaken shall have an attached proof of awards for design and/or creative services undertaken such as but not limited to copy of the certificates, photo of plaques or award received, published articles or books, etc.

Sale of Bidding Documents

Bidders who purchased bidding documents for the Solicitation No. CITEM-2018-0293 shall no longer required to purchase bid documents and offset their payment for this requirement.

This Supplemental/Bid Bulletin is being issued to clarify/modify/amend the specifications of the requirement needed, and not for purposes of delaying the proceedings of RA 9184.
