The Notice of Award shall only be issued to the lowest calculated bidder who has complied with the following eligibility documents to be submitted on or before 09 August 2018 at 11:00AM:

•       Mayor’s Permit for 2018

•       Valid PhilGEPS Registration Number (evidenced by a certificate or screen shot)

•       Notarized Omnibus Sworn Statement with Secretary Certificate (if corporation) or Special Power of Attorney (if Single Proprietorship)

Items not mentioned above remain the same.

 

This Supplemental/Bid Bulletin is being issued to clarify/modify/amend the specifications of the requirement needed, and not for purposes of delaying the proceedings of RA 9184.
