Engagement of a Certifying Body for Certification to ISO 9001:2008 of CITEM
To all Prospective bidders:
This Supplemental / Bid Bulletin is issued to clarify, modify or amend items in the Bid Documents. This shall form part of the Bid documents:
Amendments to the Terms of Reference as discussed during the Pre-Bidding Conference last May 17, 2016 as follows:
1.) III. SCOPE OF WORK/DELIVERABLES
2. 2nd Stage Certification Audit (2016)
c. Issuance of IS) 9001:2008 Certificate valid for three (3) years within one (1) to three (3)
months from the date of CITEM’s receipt of the written recommendation for certification
3. Surveillance Audit for 2nd year (2017)
a. Prepare and submit surveillance audit plan not later than two (2) weeks before the
schedules audit
4. Surveillance Audit for 3rd year (2018)
a. Prepare and submit surveillance audit plan not later than two (2) weeks before the
schedules audit
2.) V. SELECTION CRITERIA
1. The Certifying Body should be accredited for ISO 9001:2008 certification/conformity
assessment activities by a national accreditation body recognized by the Philippine
government such as the Philippine Accreditation Board or any accreditation Body
recognized by ISO/International Accreditation Forum (IAF)
3) The Certifying Body shall assess CITEM as One Site; with more or less 100 employees and all its
Management, Core and Support Processes.
Items not mentioned above remains the same.
This Supplemental/Bid Bulletin is being issued to modify/amend the specifications of the requirement needed, and not for purposes of delaying the proceedings of RA 9184.
