REQUEST FOR QUOTATION

(NEGOTIATED PROCUREMENT- SMALL VALUE PROCUREMENT)

The Center for International Trade Expositions and Missions (CITEM) invites all interested parties to submit quotation for:

 

HIRING OF HEALTH SERVICE PROVIDER TO CONDUCT THE

MANDATORY DRUG TEST (MDT) FOR CITEM EMPLOYEES

 

Approved Budget: PHP 85,000.00

 

Quotations may be submitted to the BAC Secretariat, CITEM, Golden Shell Pavilion, Roxas Blvd. cor. Gil Puyat Ave., Pasay City no later than 18 September 2018 at 11:00AM using the attached Request for Quotation (RFQ) Form duly signed by the authorized representative and with the following documents:

• List of completed projects with description
• List of past and existing clients
• Certificate of Accreditation from the Department of Health
• Photocopy of licenses and resume indicating qualifications of individual consultants / resource persons to be deployed, including educational background, years of experience, projects undertaken, client base and certifications/accreditations, if any. Only Philippine licensed physicians, nurses, medical technologists and radio technologies are allowed.

 

The Notice of Award shall then only be issued to the lowest calculated bidder who has complied with the following eligibility documents:

• Mayor’s Permit for 2018
• Valid PhilGEPS Registration Number (evidenced by a certificate or screen shot)
• Notarized Omnibus Sworn Statement with Secretary Certificate (if corporation) or Special Power of Attorney (if Single Proprietorship)

PhilGEPS Certificate of Platinum Membership may be submitted in lieu of Mayor’s Permit and PhilGEPS Registration Number.

 

CITEM-BAC reserves the right to reject any or all bids disadvantageous to, waive any formality or accept such bids as may be considered advantageous to the government. Further, CITEM-BAC assumes no obligation whatsoever to compensate or indemnify the bidders for expenses or losses that may be incurred in participating in this bidding.
