REQUEST FOR QUOTATION

(NEGOTIATED PROCUREMENT- SMALL VALUE PROCUREMENT)

The Center for International Trade Expositions and Missions (CITEM) invites all interested parties to submit quotation for:

300 PCS CUSTOMIZED USB (8GB)

Approved Budget: PHP 150,000.00

Quotations maybe submitted to BAC Secretariat, Administrative Services Division, CITEM, Golden Shell Pavilion, Roxas Blvd. cor. Gil Puyat Ave., Pasay upon payment of non-refundable fee of Two Hundred Pesos (PHP200.00) for the bidding documents.

Deadline for submission of quotation together with the following documents as required under the Implementing Rules and Regulation (IRR) Annex H Appendix A of RA 9184 should be on or before 10 October 2017 at 11:00am at the same address:

• Mayor’s Permit
• Valid PhilGEPS Registration Number (evidenced by a certificate or screen shot)
• Latest Income /Business Tax Return (ITR) duly stamped and received by BIR
• Omnibus Sworn Statement
