REQUEST FOR QUOTATION

(NEGOTIATED PROCUREMENT- SMALL VALUE PROCUREMENT)

The Center for International Trade Expositions and Missions (CITEM) invites all interested parties to submit quotation for:

FABRICATION OF 7 TROPIES AND 5 PLAQUES FOR KATHA AWARDS APRIL 2016

Approved Budget: PHP 82,500.00

Quotations maybe submitted to BAC Secretariat, Administrative Services Division, CITEM, Golden Shell Pavilion, Roxas Blvd. cor. Gil Puyat Ave., Pasay City.

Deadline for submission of quotation will be on 03 August 2017 at 11:00am at the same address.

Kindly submit your bid together with the following documents as required under   Implementing Rules and Regulation (IRR) Annex H Appendix A of RA 9184:

• Mayor’s Permit
• Valid PhilGEPS Registration Number (evidenced by a certificate or screen shot)
• Latest Income Tax Return (ITR) duly stamped and received by BIR
• Omnibus Sworn Statement

Required documents must be submitted within 3 Days after receipt of notice as the lowest bidder.
